Passport services are provided at the Council Chambers Building, 18880 SW Martinazzi Avenue, Tualatin, OR 97062
The City of Tualatin accepts passport application DS-11 and forwards it to the Regional Office of U.S. Passport Services. The City of Tualatin Passport Acceptance Agents verify the identity of the applicant by checking identification, ensure the applicant has completed the form correctly, checks that citizenship evidence is acceptable, and administers the oath to the applicant.

The City of Tualatin offers passport photo service. The charge is $16.00.

Passports & Minors
The passport Two-Parent Consent Law (Public Law 106-113, Section 236) encompasses all children under the age of 16. This means that passport agents nationwide will require the consent of both parents from children under the age of 16.
Therefore, all passport applications for minors age 0-15 will require the consent from absent parent before the passport can be issued. (FORM: DS-8053 - must be notarized and accompanied with a photocopy front and back of drivers license)
All passport applicants, regardless of age, must appear in person.

Two separate payments are required.
One payment by check or money order for the passport (payable to US Department of State)
One payment by check, money order, or credit card for the execution fee of $25 (payable to the City of Tualatin).
* Be prepared for the possibility of other fees for additional services. Fees are subject to change, for current fees please check Travel.State.Gov
** The $25.00 Execution Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form.
*** Form DS-82 not processed by the City of Tualatin. (applicant must mail directly to a Passport processing center)
